MUFON
Orange County SPEAKER AGREEMENT
This
Agreement is entered into by and between Mutual UFO Network - Orange
County, a
California nonprofit public benefit
corporation ("MUFON"), 5267 Warner Ave., PMB 275, Huntington
Beach, California 92649, and (name) ("Speaker"), (address). This
agreement will
be effective only when signed by both parties and as of the date of the
date of signature
by the last party to sign.
The
parties agree as follows:
1.
The Program. Speaker will present a program titled on (date), from
p.m. to p.m.
(Pacific
Standard or Daylight time, depending on the date). The program will be
presented
at the Costa Mesa Community Center, 1845 Park Ave., Costa Mesa,
California.
2.
Promotion and Media. To facilitate promotion of the program by
MUFON, and to
assure
adequate attendance at the program, Speaker shall provide to MUFON's
Program Chair
or Director of Public Relations, not less than six weeks prior to the
program date:
•
a biographical statement (which should include educational background,
current
occupation, degrees held, memberships in professional or related
organizations,
books written, recent radio and T.V. appearances, current
activities
or projects relating to UFO phenomena, etc.),
•
personal photo,
•
summary of the program.
•
the URL of any web site you have or which features your writings or
other
information on you.
MUFON
prefers to receive the above written material as an e-mail or e-mail
attachment
in Word or WordPerfect format. Personal photos are also preferred in
digital format, but a quality print is acceptable. Speaker consents to
taping (audio
and/or video) of the program by MUFON, at MUFON's discretion, for archive
purposes
and for sale at future MUFON programs and events.
3.
Compensation. MUFON will pay to Speaker a fee of $____at the time
of the program. Additionally,
MUFON will reimburse Speaker for the following expenses within ten days
following submission by Speaker to MUFON of reasonable expense documentation
therefor:
travel costs, one night’s lodging (may be paid directly by MUFON).
4.
Program Support. Speaker's presentation will require that MUFON
make available to Speaker
during the program the equipment checked below:

projector
___overhead projector ___laptop ___computer.
Specify any connectors, cables or other accessories

Additionally,
MUFON will provide a podium and public address system.
If
Speaker desires to sell books or tapes at the program, Speaker must advise
MUFON not
less than thirty days prior to the program date of the type and quantity
of materials Speaker
will be bringing, and whether Speaker will be providing a person to sell
such materials.
